STATEMENT OF CLAIM. This form is used in the Magistrate Court of Georgia to initiate a civil action for amounts of $15,000 or less, excluding interest and court costs. This form allows a plaintiff to file a small claim such as a suit on a note, account, or contract dispute against one or more defendants.
