The form collects detailed information about the attorney, the client, the type of legal matter involved, the total fees charged and paid, whether there was a written fee agreement, and whether the attorney informed the client of their right to fee arbitration. It also asks whether there is any ongoing litigation regarding the fee and provides space for the client to explain why they believe the charges are incorrect or excessive. The form includes a disclosure statement where the client agrees to submit to arbitration in lieu of litigation and acknowledges the final and binding nature of the arbitrators' decision, subject only to very limited grounds for appeal.
