CONFIDENTIAL DOMESTIC PARTNERSHIP RECORDS ORDER FORM. Only the registered domestic partners may obtain a copy of their confidential domestic partnership filing without a court order. All others must obtain a court order authorizing receipt of a copy of a specific confidential domestic partnership filing and must provide a certified copy of the court order with their request. (California Family Code section 298.7(a).) To obtain a copy of a confidential domestic partnership filing of record with the California Secretary of State, complete this order form, attach a check made payable to the Secretary of State for the appropriate amount or an amount “not to exceed” a specified amount written below the amount payable line, and submit your request by mail ,along with a self-addressed envelope or In person (drop off), to the Secretary of State's Sacramento office.
