CSL-118 - SIGNATURE AMENDMENT FORM. This form is used by charitable or sponsor organizations to address issues related to the signature on a previously submitted license application. It serves two purposes: Section A is completed when the person who signed the original application does not hold the official title of treasurer or chief fiscal officer but affirms that they perform the duties of that role and acted in that capacity when signing and submitting the application. Section B is used when the treasurer, chief fiscal officer, or someone performing those duties reviews the application that was filed under another signature.
