ATJ 309.1 - ORDER ON MOTION TO IMPOUND (MAKE COURT RECORDS PRIVATE) (NAME CHANGE). This is an Illinois Supreme Court–approved form used in name-change proceedings when a petitioner requests that the case file be made confidential. After reviewing the Motion to Impound, the court indicates whether public access to the record would create a hardship or negatively impact the petitioner’s health or safety. The form includes checkboxes for granting, denying, or providing specific reasons for denial, as well as space for any additional orders.
