FORM NO. FICT 4 - STATEMENT OF INTENTION TO DO BUSINESS UNDER A FICTITIOUS NAME (Foreign Entities Only). This form is used by foreign entities—businesses formed outside the State of Maine—that are authorized to transact business within the state but cannot use their legal name because it is unavailable in Maine. To comply with Maine law, such entities must adopt a fictitious name, which is a substitute name under which they may legally operate. The form requires the entity to provide its exact legal name, the chosen fictitious name, its jurisdiction of incorporation or organization, and the date it was authorized to do business in Maine.
