This form is filed pursuant to §28-3 of the Probate Act of 1975 [755 ILCS 5/28-3] and is signed by an individual who meets the legal qualifications to act in this capacity. By signing, the fiduciary agrees to protect the ward’s interests, oversee the independent administration of the estate, and either approve or object to the independent representative’s actions.
