This form is used in the Probate Division of the Circuit Court of Cook County, Illinois, by a personal representative (executor or administrator) of an estate who is not a resident of Illinois. In compliance with Section 1-11 of the Illinois Probate Act of 1975, this form allows a nonresident representative—whether newly appointed or one who becomes a nonresident during administration—to designate a resident agent within the state.
