Uniform Support Declaration

UNIFORM SUPPORT DECLARATION. This form is used in the Circuit Court of the State of Oregon as part of family law proceedings involving child support, spousal or partner support, or deviations from Oregon’s support guidelines. It is completed by either the petitioner, respondent, or another party in the case to provide the court with a full and accurate picture of their financial situation. The form collects detailed information about the number of children involved, custody arrangements (such as the number of overnights the children spend with each parent), and the party’s sources of income—including wages, benefits, and any other financial support. The form asks for information on monthly expenses such as health insurance premiums, medical costs, childcare, spousal support payments, and any government benefits received for the children. The party must also list all household fixed costs, debts, and any other financial obligations, with supporting documents such as pay stubs, tax returns, and receipts required to substantiate the reported figures. If either party is requesting to deviate from the child support guidelines set by the Oregon Administrative Rules, they must explain and provide evidence for those “rebuttal factors” as well. This declaration is signed under penalty of perjury and is used as evidence to help the court calculate or adjust the appropriate level of support.
