JD-SC-36 - APPEARANCE. This form is used in Connecticut appellate cases to officially notify the court and all parties involved that an attorney or self-represented party is entering an appearance in the case. It is also used for self-represented parties who need to update the court and other parties about a change of address. This form includes sections for the name and contact information of the individual or attorney filing the appearance, the relevant appeal and trial court docket numbers, and whether the party agrees to accept electronic service of documents in the case. If a self-represented party has changed their address, they must select the appropriate box at the top of the form, provide their new address, and file it accordingly. The form must be e-filed unless the party is exempt from electronic filing, such as in the case of incarcerated individuals. The person filing the form must certify that copies have been delivered to all other parties and that the document complies with the rules of appellate procedure.
