Description

CIV-100 PALMER - SUMMONS and NOTICE TO BOTH PARTIES OF JUDICIAL ASSIGNMENT. This form is issued by the District or Superior Court of Alaska to officially notify a defendant that a lawsuit has been filed against them. It requires the defendant to file a written answer to the accompanying complaint within 20 days of receiving the summons (or 40 days in certain circumstances, such as if the defendant is a state agency or was served outside the United States). The form provides the court's address for filing the answer and instructs the defendant to also send a copy of the response to the plaintiff or their attorney. It warns that if the defendant fails to respond within the required time, a default judgment may be entered against them. The form also includes a judicial assignment section, informing both the plaintiff and the defendant which judge has been assigned to the case. It advises all parties to keep the court informed of any changes to their contact information, and specifies different obligations depending on whether a party is represented by an attorney. Civil Rules 4, 5, 12, 42(c), 55. www.FormsWorkflow.com
