DC 8:4 - JUROR'S REQUEST TO BE POSTPONED/ EXCUSED FROM JURY SERVICE. This form is used by individuals who have been summoned for jury duty in Nebraska and wish to request either a postponement or an excuse from serving. The juror must indicate the reason for their request, such as undue hardship, extreme inconvenience, or interference with public necessity. The form allows the juror to specify the dates for which they are requesting a postponement or to be excused entirely. If the request is based on medical reasons, a doctor’s note is required. Non-medical requests may also require verification. The completed form must include the juror’s personal and contact information, and must be signed and dated before being submitted to the court for consideration. Neb. Rev.
