JD-1754 - NOTICE OF CHANGE OF PLACEMENT. This form is used in the Wisconsin Circuit Court to formally notify all parties of a change in placement for a child or juvenile who is currently under either a temporary physical custody order or a dispositional order. It is completed by a caseworker or district attorney/corporation counsel and outlines the new or proposed placement address, the type of placement change (such as out-of-home to in-home), and whether the change is occurring due to emergency circumstances or was authorized in a prior court order.
