JD-1754 - NOTICE OF CHANGE OF PLACEMENT. This form is used in the Wisconsin Circuit Court to formally notify all parties of a change in placement for a child or juvenile who is currently under either a temporary physical custody order or a dispositional order. It is completed by a caseworker or district attorney/corporation counsel and outlines the new or proposed placement address, the type of placement change (such as out-of-home to in-home), and whether the change is occurring due to emergency circumstances or was authorized in a prior court order. The form also provides an explanation for why the new placement is preferable and how it aligns with the child's treatment or permanency plan. If the placement change is made under emergency conditions, the notice must be filed within 48 hours. Recipients of the notice have the right to file a written objection within 10 business days, which would prompt the court to schedule a hearing.
