JD-1792 - ORDER FOR CHANGE IN PLACEMENT (OUT-OF-HOME TO IN-HOME PLACEMENT ONLY). This form is used by the Wisconsin Circuit Court to officially document and authorize a change in the placement of a child or juvenile from an out-of-home setting back into an in-home placement. It applies when the child or juvenile is already under a temporary physical custody order or a dispositional order. The form indicates whether a notice of the proposed placement change was filed, whether objections were raised, and whether a hearing occurred. Upon the court's review and findings, the judge orders the in-home placement and, if applicable, specifies who is responsible for transportation and the expiration date of the dispositional order.
