IW-1788 - ORDER FOR EXTENSION OF DISPOSITIONAL ORDER/CONSENT DECREE (OUT-OF-HOME PLACEMENT ONLY) – INDIAN CHILD WELFARE ACT. This form is used by the Circuit Court in the State of Wisconsin to document and order the extension of either a dispositional order or a consent decree for a child or juvenile who is placed out of the home, specifically under the jurisdiction of the Indian Child Welfare Act (ICWA). It is completed following a request for extension and either the absence of objections or a court hearing.
