IW-1788T - ORDER FOR EXTENSION OF DISPOSITIONAL ORDER/CONSENT DECREE WITH TPR NOTICE (OUT-OF-HOME PLACEMENT ONLY) – INDIAN CHILD WELFARE ACT. This form is used in the Wisconsin Circuit Court to extend either a dispositional order or a consent decree for a child or juvenile who is placed out of their home under the Indian Child Welfare Act. The court reviews a filed request for extension and determines whether to grant or deny it based on findings such as whether the allegations supporting the extension are proven and if reasonable efforts were made to achieve the permanency goals in the child's permanency plan. The form documents whether reasonable efforts were made to keep siblings together and if active efforts were provided to prevent the breakup of the Indian family. Parents who appeared in court are orally advised about the grounds for termination of parental rights (TPR) and the conditions necessary for the safe return of the child or restoration of visitation rights. The court orders may include setting an expiration date for the extension based on the child's age, schooling status, or other criteria, continuing supervision or placement, and provisions for sibling visitation.
