JC-1665 - ORDER FOR POST-TERMINATION OF PARENTAL RIGHTS CHANGE IN PLACEMENT. This form is used by the Circuit Court in the State of Wisconsin to document and authorize a change in placement for a child who is under guardianship following the termination of parental rights. It is applicable when a child is already subject to a guardianship order under specific statutory provisions (§48.427(3m)(a)1.-4., (am), or (b), Wis. Stats.). The form records whether the court approves the proposed placement change—either based on agreement or following a hearing—and whether the recommendation of the responsible department or agency is adopted.
