JD-1806 - REQUEST FOR TRANSITION TO DISCHARGE HEARING. This form is used in Wisconsin circuit courts to request a Transition to Discharge Hearing for a child or juvenile placed in out-of-home care. It applies to cases where the child is a full-time secondary school student with an individualized education program in effect, and the child has expressed a desire to discharge from out-of-home care upon the termination of the dispositional order. The purpose of the hearing is to inform the child or juvenile of their options before the dispositional order ends, including discharging from care, continuing placement through an extension of the order, or re-entering care under a Voluntary Transition to Independent Living Agreement before receiving a diploma or reaching age 21.
