JD-1766 - REQUEST TO CHANGE PLACEMENT / REVISE DISPOSITIONAL ORDER. This form is used in Wisconsin circuit court to request either a change in the placement of a child or juvenile or a revision of the existing dispositional order in a case. The person filing the form identifies their role in the case, such as caseworker, guardian ad litem, parent, or child’s counsel. If the child is an Indian child, a different form must be used. The form allows the filer to specify the current custody or dispositional order and detail the proposed change in placement—whether from in-home to out-of-home care, out-of-home to another placement, or other variations. The filer must provide the name and address of the proposed new placement, the date for the proposed change, and explain the reasons why the new placement is preferable and how it aligns with the child’s treatment or permanency plan. If the placement involves removal from the home, the form requires statements regarding reasonable efforts made to prevent removal, or whether such efforts were not required. If applicable, the form also covers whether the proposed placement is a Qualified Residential Treatment Program and whether a qualified individual’s assessment is attached or forthcoming. Additionally, the filer may request the court to revise the dispositional order, either based on new information or signed waivers of objection, and can request a hearing on the matter.
