IW-1766 - REQUEST TO CHANGE PLACEMENT / REVISE DISPOSITIONAL ORDER INDIAN CHILD WELFARE ACT. This form is used in Wisconsin Circuit Court when requesting a change in placement or a revision to a dispositional order involving a child or juvenile who is subject to the Indian Child Welfare Act (ICWA).
