CCP 0226 - ORDER APPOINTING STANDBY GUARDIAN AS PLENARY GUARDIAN OF DISABLED PERSON. This form is a court order used by the Probate Division of the Circuit Court of Cook County, Illinois, when the previously appointed plenary guardian of a disabled adult is no longer able to serve. This order authorizes a standby guardian—who was previously approved and designated by the court—to assume full guardianship responsibilities. A "plenary guardian" is granted comprehensive authority over the person, the estate, or both, depending on what the court specifies in the order. Once appointed, the new guardian must comply with certain statutory obligations, including filing an inventory of the disabled person’s assets within 60 days and submitting annual verified accounts (for guardians of the estate) or reports (for guardians of the person) as required under the Illinois Probate Act.
