CCP 0220 - ORDER EXTENDING TEMPORARY GUARDIANSHIP OF ALLEGED PERSON WITH A DISABILITY. This form is a court order issued by the Probate Division of the Circuit Court of Cook County, Illinois. This form is used when a petitioner seeks to extend the temporary guardianship of an individual who is alleged—but not yet legally adjudicated—to be a person with a disability. Temporary guardianship is initially granted to protect the alleged disabled person from immediate harm or risk, and this order allows for that protection to be continued beyond the original period. The form is used when a previous court order has already appointed a temporary guardian for the person, estate, or both, and a Petition for Plenary or Limited Guardianship under §11a-8 of the Illinois Probate Act is still pending. The court must find that extending the temporary guardianship is in the best interest of the alleged disabled person and that there is a risk of abuse, neglect, self-neglect, exploitation, or other harm if guardianship is not continued. The order requires the petitioner to explain why the extension is necessary and to describe the specific harm that justifies it. The temporary guardianship may be extended up to a maximum of 120 days from the date of the original appointment, unless the guardianship is terminated earlier by court order.
