Description
MV-900 - NOTICE OF LIEN. This New York DMV form is used to officially record a lender’s security interest in a vehicle, boat, trailer, or manufactured home with the Department of Motor Vehicles. It collects identifying information about the asset (VIN/HIN, year, make, body/hull type), the owner(s) (names, ID numbers, and address), and the lienholder (name, lien filing code, and mailing address), and requires original signatures from both the owner(s) and the lienholder, plus the date of the security agreement. If a New York title has already been issued, the owner must provide it so DMV can issue a new Certificate of Title showing the lienholder; if not, the owner’s name must match DMV records exactly. The lienholder submits the completed form, the $5 fee, and any existing title to the DMV Title Bureau. www.FormsWorkflow.com
