Description
CC-DC-PO-006 - PETITION TO MODIFY; RESCIND; EXTEND PEACE ORDER. This form is used in the Circuit Court or District Court of Maryland by either the petitioner or respondent involved in an existing Peace Order case. It allows the filing party to request a change to the terms of a previously issued Peace Order. The form can be used to ask the court to modify the order (by changing its conditions), rescind it (cancel it altogether), or extend it (continue its effect for a longer period), depending on the circumstances. The petitioner must provide specific reasons and factual details supporting the request, such as incidents that occurred, dates, locations, and any injuries sustained. The form also requires personal and contact information, a declaration under penalty of perjury that the statements made are true, and a Certificate of Service to confirm that the opposing party has been properly served with a copy of the petition. If the petition includes any confidential or restricted information, it must be accompanied by a Notice Regarding Restricted Information (form MDJ-008). www.FormsWorkflow.com
