SOS FORM NP 005 - APPLICATION FOR NOTARY PUBLIC CHANGE OF NAME. This form is used by notaries public to officially notify the Mississippi Secretary of State of a legal name change. This form is required when a notary's name changes due to marriage, divorce, court order, or other legal reasons. The notary must provide documentation supporting the name change, such as a marriage certificate, divorce decree, or court order, along with a bond rider from their bonding company to amend their existing notary bond. Additionally, if the notary is also changing their address, they may update their contact information on the form. The updated name will appear on the notary’s replacement commission.
