SOS FORM NP 006 - APPLICATION FOR REPLACEMENT COMMISSION. This form is used by notaries public to request a replacement of their notary commission certificate or notary seal in cases where these items have been lost, stolen, or destroyed. Notaries must specify the reason for requesting a replacement and, if the commission certificate or seal was stolen, they must attach a copy of a police report. Additionally, if the notary’s name has changed, legal documentation such as a marriage certificate, court order, or divorce decree must be provided, along with a bond rider from their bonding company amending the notary bond. Notaries may also update their contact or business information using this form. Once processed, the replacement commission certificate will be emailed to the notary’s address of record. The form must be completed, signed, and submitted along with a $20.00 fee to the Mississippi Secretary of State’s Office. www.FormsWorkflow.com
