Form 3DC47 - RETURN OF SERVICE; ACKNOWLEDGMENT OF SERVICE. This form is used in the District Court of the Third Circuit in Hawai‘i to document and verify that legal documents have been properly delivered (served) to a party in a civil case. This form is completed by the person serving the documents—such as a law enforcement officer, process server, or qualified individual not involved in the case—and includes details such as the name of the party served, address, date and time of service, mileage, and method of service (personal, substitute, business, or garnishment).
