NOT103 - PERMANENT COMMISSION NOTARY PUBLIC APPLICATION. This form is used by attorneys licensed to practice law in Wisconsin to apply for a permanent notary public commission through the Wisconsin Department of Financial Institutions. Unlike the standard four-year commission, this permanent commission is available only to individuals who are United States residents, actively licensed in Wisconsin, and have never had their law license suspended, revoked, or surrendered for any reason. Non-attorneys or attorneys with disciplinary histories must instead apply for a standard four-year commission. Applicants must first obtain a Certificate of Good Standing from the Wisconsin Supreme Court, confirming their bar status, and ensure it is dated within two months of their application. Before submitting the application, they must also purchase a notary seal or stamp bearing their printed name (with full last name), the phrase “Notary Public,” and “State of Wisconsin.” This seal or stamp must match the official notary signature used on all documents. The form requires personal details such as name, address, date of birth, phone number, email address, and state bar number. The applicant must sign a declaration affirming their qualifications and submit a clear impression of their notary seal or stamp.
