DL-015 - CERTIFIED/NON-CERTIFIED DRIVER RECORDS REQUEST PROCEDURE. This form from the Maryland Department of Transportation Motor Vehicle Administration is used to request multiple driving records—five or more—pertaining to individuals who hold Maryland driver’s licenses. The form outlines specific formatting and submission instructions that must be followed to ensure the request is processed correctly. This includes requirements such as using 8 1/2” x 11” white paper, submitting three copies of the list, double-spacing license numbers or personal information, and limiting each page to 15 requests. Only Maryland driver license numbers, which follow a specific format (e.g., A-000-000-000-000), may be used in the license column. If the license number is not provided, the request must include the full name and date of birth of the individual. Each request must be accompanied by a prepaid fee—$12.00 for a non-certified record or $15.00 for a certified record—paid via check or money order. Incomplete or improperly formatted requests will not be processed. The form also requires the requestor’s signature, reason for the request, and agreement to use the information only for authorized purposes, in compliance with the Maryland Public Information Act.
