JD-1780 - PETITION TO EXPUNGE COURT RECORD OF ADJUDICATION/RECOMMENDATION OF DISTRICT ATTORNEY. This form is used in Wisconsin Circuit Court for a person who was previously adjudicated delinquent as a juvenile to request that the court expunge the record of that adjudication. The petitioner, who must now be at least 17 years old, declares under oath that they have successfully completed all the terms and conditions of the court’s dispositional order in the case and that expunging the record will benefit them without causing harm to society. The form makes clear that the expungement only applies to the court’s record of adjudication and does not affect records maintained by law enforcement, the Crime Information Bureau, district attorney’s offices, or social services agencies. The petitioner must also obtain a recommendation from the district attorney who handled the case.
