CC-DC-DV-016 - REQUEST TO REGISTER OUT-OF-STATE ORDER FOR PROTECTION (Family Law § 4-508.1). This form is used in the Circuit or District Court of Maryland to formally request the registration of a protective order that was originally issued by a court in another state. This form allows an individual (the petitioner) to ask the Maryland court to recognize and enforce an out-of-state order that was issued to prevent violence, threats, harassment, contact, or proximity by a respondent. The petitioner must provide information about the original order, including the names of the parties involved, the issuing court, the case number, and the expiration date of the order. A copy of the out-of-state order must be attached to the request. The petitioner must also affirm that the original order was issued by either a civil or criminal court in response to a formal request for protection and specify whether their contact information was or should be disclosed to the respondent. www.FormsWorkflow.com
