This form is used in Maryland by a Respondent who has filed a request to shield (seal or remove from public view) records related to a protective order and is unable to serve a copy of that request on the original Petitioner due to the Petitioner’s confidential address. Because service of legal documents is normally required to ensure all parties are notified, this form allows the Respondent to ask the court to authorize the court clerk to carry out service on their behalf.
