Description
CC-DR-079 - REQUEST TO REGISTER AN OUT-OF-STATE CHILD CUSTODY ORDER (Md. Code, Family Law Art., § 9.5-305). This form is used in the Maryland Circuit Court by an individual seeking to register a custody order that was issued by a court in another state. Registration allows the Maryland court to recognize and, if necessary, enforce the existing custody or visitation (child access) order as if it were a Maryland order. The petitioner must provide the names and birthdates of the children involved, the details of the out-of-state order (including the court name, state, date, and case number), and the names and addresses of the individuals granted custody or visitation. The form also requires the petitioner to affirm that the custody order has not been modified. Two copies of the out-of-state order—one of which must be certified—must be attached. www.FormsWorkflow.com
