CC-DR-004 - COMPLAINT FOR CUSTODY (Md. Code, Family Law Art., § 5-203). It is filed under Maryland Code, Family Law Article, § 5-203. The person(s) filing the complaint must identify their relationship to the child(ren), list all parties involved, and provide detailed information about the child(ren)’s living arrangements, prior residences, and any other court cases involving the child(ren), including custody, support, or protection orders. The form also requires disclosure of any non-party individuals who currently have physical custody or claim custody or visitation rights. The filer(s) must indicate whether they are seeking joint or sole legal and/or physical custody, explain why such custody is in the best interest of the child(ren), and provide details on any proposed visitation schedule. If applicable, the complaint may also request orders for child support, health insurance coverage, or other relief related to the child(ren)’s care.
