JN-1514 - STATEMENT BY PROPOSED GUARDIAN. This form is used in the State of Wisconsin Circuit Court to provide information about an individual who has been nominated to serve as the guardian of a child. It must be submitted to the court at least 96 hours before the initial guardianship hearing. The proposed guardian is required to disclose key details under oath, including their relationship to the child, the number of dependents they already care for, their ability to financially support the child, any criminal history, and whether they have been found to have abused or neglected a child.
