Description

JD-1760 - PETITION FOR JUDGMENT AGAINST JUVENILE/PARENT FOR UNPAID FORFEITURE/SURCHARGE. This form is used in the Wisconsin Circuit Court to formally request that the court enter a civil judgment against a juvenile or their custodial parent for failing to pay a court-ordered forfeiture or surcharge. The petitioner, who may be a district attorney, corporation counsel, law enforcement representative, or other designated official, provides details of the unpaid financial obligation initially ordered by either this court or a municipal court. The petition includes the amount still owed, the identity of the party responsible for payment, and the expiration date of the court order that imposed the obligation. It must be filed within one year of the expiration of that order. The petitioner asks the court to schedule a hearing and issue a judgment in favor of the county or municipality to collect the unpaid amount. §895.035(2m)(b), Wisconsin Statutes. www.FormsWorkflow.com
