JD-1725 - NOTICE TO SCHOOL BOARD. This form is used by the Wisconsin Circuit Court to formally notify a school district about specific developments in a juvenile delinquency or child welfare case involving a student enrolled in that district. It may be used to inform the school board that a petition has been filed alleging the juvenile committed an act that would be considered a felony if committed by an adult, or to report that such a petition has been dismissed. It may also be used to notify the school that the juvenile has been adjudicated delinquent of either a misdemeanor or felony, including offenses committed for the benefit of a gang, and that the juvenile has since transferred into the school district. In some cases, the form also communicates that the juvenile is subject to a court order requiring school attendance as a condition of a dispositional order under CHIPS (Children in Need of Protection or Services), JIPS (Juveniles in Need of Protection or Services), or delinquency proceedings. The notice may include attached dispositional orders and identifies the county department responsible for supervising the juvenile.
