VOLUNTARY ACKNOWLEDGMENT OF PATERNITY (A.R.S. §25-812). This form is used in the Superior Court of Arizona, Mohave County and allows parents to legally establish paternity without a court trial. By signing, both parents confirm the identity of the child's biological father, which enables the Office of Vital Records to amend the child's birth certificate to reflect the father’s name and, if requested, change the child’s last name. The form provides options for proving paternity through either an Affidavit of Acknowledgment or genetic (DNA) test results. Once submitted, the court issues an official order granting the father legal parental rights and responsibilities, though it does not determine child support, custody, or parenting time. Parents are advised of their right to seek legal counsel before signing, and the acknowledgment may be rescinded under certain conditions as outlined in A.R.S. § 25-812(H).
