FL Divorce 202 - PETITION TO END REGISTERED DOMESTIC PARTNERSHIP (DISSOLUTION) (PTDSS). This form is used in Washington state for individuals seeking to dissolve a domestic partnership or civil union that was registered with the state. Use this form only if your domestic partnership or civil union was registered with a state (not a city or county). In this form, the term “domestic partnership” includes domestic partnerships and civil unions. Do not use this form if you are married or have a state registered domestic partnership that converted into a marriage by law on June 30, 2014. Use the Petition for Divorce (form FL Divorce 201) instead. The form includes sections to provide information about both parties (petitioner and respondent), the domestic partnership, the request to end the partnership, and jurisdiction over the partners. It asks if one of the partners is pregnant and provides information on children involved, including their ages and where they’ve lived.
