This form is used for a Limited Liability Company (LLC) in the State of Maine by LLCs organized in another jurisdiction (foreign LLCs) to qualify to conduct business in Maine. The form collects information such as the name of the LLC, the name it will use in Maine if different from its original name, date of formation, address of the principal office, nature of business or purposes in Maine, details about the registered agent, information about managers (if any), and the date the LLC commenced or expects to commence conducting activities in Maine. Additionally, it requires an original signature of at least one authorized person and must be signed under oath or affirmation.
