The form is a legal form used in the Maryland Circuit Court when a biological parent voluntarily agrees to terminate their parental rights in an independent adoption—meaning an adoption that is not facilitated by a public or private adoption agency. This form is appropriate only for a parent whose rights are being fully relinquished, not for cases such as step-parent adoptions where one parent retains parental rights. The form outlines a parent's right to receive the consent form and instructions in a language they understand, to consult with a lawyer (especially if under 18 or with limited understanding), and to receive adoption counseling and guidance.
