Description
NCM-R 2003.5 - REQUEST FOR NAME CHANGE (Minor Name Change). This is a legal form used to request a name change for a minor in Illinois. The parent or guardian fills out the form with the current and proposed new names of the children. After filing the form with the Circuit Court Clerk’s office in the county where they live, they must publish a notice of the name change in a local newspaper once a week for three weeks. They must also serve notice of the name change to the other parent unless they qualify to waive notice. They will need to attend a hearing in front of a judge who will review the petition and ask questions about the name change. The form warns that making a false statement on this form is perjury and has penalties provided by law. It also mentions that there are usually fees to file court forms like this one, but some may qualify to file for free. The form takes about 30-45 minutes to fill out. www.FormsWorkflow.com
