APPLICATION TO REGISTER FOR CM/ECF AS SELF-REPRESENTED PARTY. This form is used by non-incarcerated individuals who are representing themselves in a case before the U.S. District Court for the District of Oregon to request access to the court’s Case Management/Electronic Case Files (CM/ECF) system. If approved, this access allows the applicant to file documents electronically and receive service of court filings via email, which streamlines case management and communication. Applying for a CM/ECF user account constitutes consent to receive electronic service of documents. However, the Court must approve the application before e-filing privileges are granted, and these privileges may be revoked at any time. This form also provides an option to opt out of electronic service if the application is denied. Applicants must certify they have reviewed the CM/ECF and PACER user manuals and understand the responsibilities involved.
