This form is required when there is a change in the entity’s legal name, jurisdiction of formation, street address, or registered agent information. Additionally, if a Foreign Limited Liability Company (LLC) wishes to become a Foreign Master LLC, or vice versa, this form must be completed along with any necessary supporting documents. The amendment ensures that the state has up-to-date and accurate information about the business.
