This form is used by an attorney or law firm to notify a client (or the client's authorized agent) of the attorney's intent to file a lien for unpaid legal services. This lien is placed on the client's real property to secure payment for the services rendered. The form includes the amount of the lien, the legal description of the property, and the date when the last legal service was performed. It is filed in accordance with Minnesota Statutes, specifically Minn. Stat. 481.13, and must be acknowledged by a notary.
