NOTICE OF ASSIGNMENT TO A MAGISTRATE JUDGE AND CONSENT/REQUEST FOR REASSIGNMENT. This form is used in the United States District Court for the Western District of Wisconsin when a civil case has been randomly assigned to a United States magistrate judge. It allows parties in the case to either consent to have the magistrate judge conduct all proceedings, including trial and final judgment, or to request reassignment to a district judge. The process requires each party to complete and email the attached Notice of Assignment to a Magistrate Judge and Consent/Request for Reassignment to the court at a designated email address at least two weeks before the preliminary pretrial conference. The form should not be electronically filed or served on opposing counsel. If all parties consent, the case remains with the magistrate judge. If even one party requests reassignment, the case will be randomly reassigned to an Article III district judge.
