This form is used in the United States District Court for the Western District of Wisconsin to help judges evaluate potential conflicts of interest that could require their disqualification or recusal. It must be submitted by counsel for any private (non-governmental) business, company, or corporation when making their first filing in a case, such as a pleading, motion, or response.
