This form is used by Oregon notaries to report and request action when their notary stamp, certificate of authority to obtain a stamp, or notary journal is lost, misplaced, stolen, or otherwise unavailable. The notary affirms the loss and provides details about it, requests a new Certificate of Authorization to obtain a new stamp, and can update their email address. If the journal is lost or stolen, they must report when it was discovered missing. The notary must notify the Secretary of State within 10 days if the missing items are found.
