GU-2 - ORDER FOR INVESTIGATION OF NON-RELATIVE GUARDIAN(S) BY HEALTH & HUMAN SERVICES, CHILD WELFARE DIVISION. This form is used in the Superior Court of California, County of Napa, in cases involving the guardianship of minor(s). It directs the Health & Human Services, Child Welfare Division to conduct an investigation and file a report before the hearing on the Petition for Appointment of Guardian(s) of Minor(s). The division is also instructed to screen the proposed guardian(s) for prior referrals of neglect or abuse of the minor, perform a Foster Family Home Licensure screening, and file a written report with the court detailing the investigation results at least 5 days before the hearing. Additionally, the petitioner is required to mail a notice of the hearing and a copy of the petition to Health & Human Services, Child Welfare Division at least 15 days prior to the hearing.
