This form is used in Solano County civil cases when a defendant has been personally served, failed to appear or respond within the time allowed by law, and the plaintiff seeks a court-entered default judgment. Unlike a clerk-entered default for contract damages only, this form reflects a judgment issued by a judge after a hearing and consideration of evidence.
